Our featured speaker, Dr. Jonathan Hart, brings a wealth of experience to his role of VP of Clinical Strategy, Population Health at Lee Health, including stints as a Rural Family Physician, an ED Physician and a Hospitalist. He worked for six years as Chief Medical Officer for a Commercial Payer then joined Lee Health in 2015 to build the Physician Advisor Program for Care Management and was quickly made accountable for all Utilization Management and Care Management for the System. In his current role, he oversees the strategy, clinical operations, integration, and effects of the clinical aspects of Population Health and Value-based Care efforts for Lee Health.
Dr. Hart’s expertise will shine a light on the often misunderstood concepts that form the foundation of population – and he will offer actionable insights for what other leaders can learn from Lee Health’s journey towards managing Population Health.
